The following items are required to participate in KCSD activities (grades 7-12) as well as requirements of The Iowa High School Athletic Association and the Iowa Girls’ High School Union.  Student athletes cannot practice or compete in activities until these forms are received.

1) Required annually: 
Physical evaluation (please submit to Email Ryan Paulsen, fax to 641-842-2066 or hand to the coach on the first day of practice)
-A physical examination is valid for one calendar year plus one month.
2) Required annually: 
Parents/guardians must annually acknowledge the receipt of the "HEADS UP: Concussion in High School Sports” facts sheet.  If this information was acknowledged by a parent/guardian during online student registration (Infinite Campus), a paper copy is not necessary.
3) Required once during 7-12th KCSD athletics: 
Consent for medical treatment (please submit to Email Ryan Paulsen, fax to 641-842-2066 or hand to the coach on the first day of practice). This form is completed during school registration on Infinite Campus.
